What is Erectile Dysfunction?

Erectile dysfunction refers to ongoing difficulty achieving or maintaining an erection sufficient for sexual activity. Erections involve several body systems, including blood vessels, nerves, hormones, and psychological factors. Changes involving one or more of these areas can contribute to ED.

Several factors can play a role in erectile dysfunction. Your medical history, previous treatments, medications, and other aspects of your health will be considered during your evaluation with Dr. Coutinho. Blood pressure, testosterone levels, metabolic health, smoking history, and cardiovascular risk may be important parts of the discussion.

An evaluation can help identify possible causes and determine which treatment options may be appropriate for your individual situation. An evaluation of penile blood flow via an in-office ultrasound (Penile Doppler study) can be useful to distinguish between various types of erectile dysfunction and aid in deciding treatment options.

How Is Erectile Dysfunction Treated

Treatment for erectile dysfunction is selected according to the possible cause of your symptoms, your overall health, previous treatment, and individual preferences. Dr. Coutinho will begin with an evaluation and discuss which options may be appropriate for you.

Depending on your needs, treatment may involve lifestyle and cardiovascular risk-factor modification, oral PDE5 inhibitor medications, vacuum erection devices, intraurethral or injection therapy, treatment of a documented hormonal abnormality, or surgical treatment with a penile prosthesis.

Inflatable Penile Prosthesis

An inflatable penile prosthesis is a surgically implanted device that may be considered for men whose erectile dysfunction has not responded adequately to other treatments or who prefer a surgical option. These devices come in 2- and 3-piece varieties, and use a completely internal system that allows for erections without pills or injections. The device is hidden entirely inside the body with no external parts.

The device is placed inside the body and can be inflated when an erection is desired and deflated afterward. Dr. Coutinho is experienced in penile prosthetic surgery, with reconstructive and prosthetic training relevant to complex implant placement. He can explain how the implant works, what surgery involves, and whether this treatment may be appropriate based on your condition and previous care.

During your consultation, Dr. Coutinho will review the available operative approaches and develop a treatment plan based on your individual needs.

Related Men’s Health and Urologic Care

Erectile dysfunction can occur alongside other urinary or men’s health concerns. During your evaluation, you can discuss these symptoms with Dr. Coutinho so he can determine whether separate testing or treatment may be appropriate.

Some men with erectile dysfunction also experience urinary incontinence, including after certain prostate procedures. An artificial urinary sphincter is a surgically implanted device used to treat certain forms of male urinary incontinence. Dr. Coutinho can evaluate urinary leakage separately and discuss whether treatment is appropriate.

Who Are the Best Candidates for Erectile Dysfunction Treatment?

Men experiencing ongoing changes in erectile function may benefit from an evaluation with Dr. Coutinho. Determining the appropriate treatment begins with understanding the possible cause of the problem and any health factors that may be contributing.

What to Expect on the Day of Erectile Dysfunction Treatment

What you can expect will depend on the treatment selected. Some erectile dysfunction treatments involve medication or other nonsurgical therapies and do not require a surgical procedure.

If you are undergoing inflatable penile prosthesis surgery, the care team will review your surgical plan before treatment, and anesthesia will be used during the procedure. Dr. Coutinho will place the components of the inflatable device according to the planned surgical approach.

After surgery, you will receive instructions on incision care, medications, activity, follow-up, and when you can use the device. Dr. Coutinho will also explain how to operate the prosthesis as part of your postoperative care.

Your Guide to Post-Erectile Dysfunction Treatment Care

Recovery and follow-up will depend on the type of erectile dysfunction treatment you receive. Nonsurgical therapies may require little or no physical recovery, although Dr. Coutinho may schedule follow-up care to evaluate your response to treatment.

Surgical treatment with an inflatable penile prosthesis requires a healing period before the device can be used. Dr. Coutinho will provide instructions regarding incision care, medications, activity, and other precautions based on your procedure.

Follow-up visits allow him to monitor your recovery and provide guidance about using the implant when appropriate.
