Prostate Enlargement

Improve Urinary Flow & Comfort

Introduction to Prostate Enlargement Treatment

An enlarged prostate can place pressure on the urethra and interfere with the normal flow of urine. This can lead to symptoms such as a weak urine stream, frequent or urgent urination, difficulty emptying the bladder, and waking during the night to urinate.

At East Coast Advanced Urology in Clifton, NJ, Dr. Karl Coutinho offers several treatment options for benign prostatic hyperplasia (BPH). He welcomes patients from throughout the greater New York metropolitan area and recommends treatment based on each patient’s symptoms, prostate condition, overall health, and previous treatment.

What is Prostate Enlargement?

Prostate enlargement, also called benign prostatic hyperplasia (BPH), is a noncancerous enlargement of the prostate gland. The prostate surrounds part of the urethra, which carries urine out of the bladder. As the prostate becomes larger, it can narrow the urethra and interfere with urine flow.

As BPH begins to affect urine flow, you may notice changes in your usual bathroom habits. These can range from trouble getting started or maintaining a steady stream to needing to urinate more often, particularly at night. Some men also have difficulty fully emptying their bladder.

BPH is different from prostate cancer. Dr. Coutinho can evaluate your symptoms and prostate health to determine what may be causing your urinary concerns and whether treatment is appropriate.

How to Treat Prostate Enlargement?

Treatment for an enlarged prostate depends on factors such as your symptoms, prostate anatomy, overall health, previous treatment, and personal priorities. Some patients may be managed with medication or monitoring, while others may benefit from a procedure to address urinary obstruction.

A guideline-based BPH evaluation commonly includes a symptom assessment and urinalysis. Depending on the treatment being considered, Dr. Coutinho may also measure post-void residual urine, urine-flow rate, and prostate size or anatomy using imaging or cystoscopy. The right approach to prostate enlargement will vary from one patient to another. Dr. Coutinho will use the findings from your evaluation to recommend an appropriate treatment plan.

Dr. Coutinho offers several treatment options, including:

UroLift® is a minimally invasive procedure that uses small implants to pull enlarged prostate tissue away from the urethra. This creates more room for urine to pass without cutting or removing prostate tissue. It is suitable for certain prostate sizes and prostate architecture. Dr. Coutinho can determine whether your prostate anatomy and symptoms make UroLift an appropriate option.

Who Are the Best Candidates for Prostate Enlargement Treatment?

Not everyone with an enlarged prostate requires the same treatment. Dr. Coutinho will evaluate your urinary symptoms, prostate, medications, overall health, and previous treatment before recommending an appropriate approach.

Patients who may benefit from treatment include men who:

  • Have bothersome urinary symptoms associated with BPH
  • Experience a weak or interrupted urine stream
  • Have difficulty completely emptying the bladder
  • Need to urinate frequently or urgently
  • Continue to experience symptoms despite medication or other management
  • Have experienced urinary retention or other problems associated with prostate enlargement
  • Want to discuss procedural options for managing their symptoms

What Are the Benefits of Prostate Enlargement Treatment?

The purpose of treatment is to address the urinary problems associated with an enlarged prostate. The specific changes you experience will depend on your symptoms and the treatment Dr. Coutinho recommends.

Treatment may help with:

  • Urine flow
  • Bladder emptying
  • Frequent trips to the bathroom
  • Urinary urgency
  • Difficulty starting urination
  • Nighttime urination

What to Expect on the Day of Prostate Enlargement Treatment

What happens on the day of treatment will depend on the procedure Dr. Coutinho recommends. Before treatment begins, he and the care team will review your procedure, preparation instructions, and what you can expect afterward.

UroLift®, GreenLight™ laser prostate surgery, and TURP use different methods to address urinary obstruction caused by an enlarged prostate. Dr. Coutinho will explain how your specific procedure is performed, the type of anesthesia or numbing medication that may be used, and whether a urinary catheter may be needed afterward.

Before you leave, you will receive instructions regarding medications, activity, urinary symptoms, and follow-up care based on your procedure.

Other Procedures and Evaluations

Urinary symptoms can have more than one possible cause. Depending on your symptoms and testing, Dr. Coutinho may recommend additional evaluation or treatment as part of your urologic care.

Cystoscopy

Cystoscopy allows Dr. Coutinho to examine the inside of the urethra and bladder using a small camera. It may be used to evaluate urinary symptoms further, examine the urinary tract, or gather information to help guide treatment.

Cystolitholapaxy

Difficulty emptying the bladder can contribute to bladder stone formation in some patients. Cystolitholapaxy is used to break bladder stones into smaller pieces that can be removed through the urinary tract. If bladder stones are present along with prostate enlargement, Dr. Coutinho can determine how both conditions should be addressed.

Prostate Biopsy (MRI Fusion)

An abnormal PSA, prostate examination, or imaging finding may require additional evaluation separate from BPH treatment. When appropriate, an MRI fusion prostate biopsy can be used to collect tissue from areas of the prostate that require closer examination.

Prostate Enlargement Treatment Recovery

Your Guide to Post-Prostate Enlargement Care

Recovery will depend on the type of prostate enlargement treatment you receive and your individual response to the procedure. Dr. Coutinho will explain what to expect based on the treatment selected for you.

You will receive instructions regarding medications, fluids, activity, sexual activity, and other precautions during recovery. If a urinary catheter is needed, Dr. Coutinho will also explain how to care for it and when it may be removed.

Follow-up visits allow Dr. Coutinho to monitor your recovery, evaluate your urinary symptoms, and determine whether any additional care is needed.
